Funding Requests

Complete the appropriate request form and send to chapter leadership board for approval. If you are not sure which form to use, please contact your chapter leadership board:

  • For fundraisers and hands-on service projects, complete the Care Abounds in Communities Request for Funding. (PDF, 754K)
  • For educational and member social activities, complete the Operating Funds Request form. (PDF, 750K)

Permission to Disclose Information

Recipients (those who receive the help) must complete this form if the chapter and service team want to disclose/publish recipient information for the activity.

Logo Request Form

Complete and submit if you need to use the Thrivent Financial for Lutherans logo. Please note that artwork containing the logo needs to be reviewed before the logo is applied.

Permission to Disclose Information – Regular

Recipients (those who receive the help) must complete a "Permission to Disclose Information" form. This completed form provides Thrivent Financial for Lutherans with permission to use, disclose and publish the recipient's name and other pertinent information about the recipient for the purposes of planning, conducting, advertising and communicating the results of a fund-raising activity for the recipient's benefit (e.g., to publicize the fund-raising activity).

Note: If the recipient is a qualified nonprofit organization, the form does not need to be signed unless a person's name or likeness is published, which could be an employee or volunteer of the nonprofit organization.
