Every day, Thrivent Financial for Lutherans members come together to conduct an average of 275 activities in their communities nationwide – from packing food for the hungry to holding benefit concerts to sponsoring a seminar about values and money.

Thrivent Financial provides tools to help our members successfully plan, promote, conduct and report these activities with and through our chapters.

Confidentiality and Privacy

Both Thrivent Financial and its chapter leaders have a responsibility to respect and uphold the confidentiality and privacy of those who entrust us with personal information. Not only is this practice mandated, it is the right thing to do.

With this in mind, be sure to keep all personal information private and take steps to retain confidentiality (e.g., not sharing member lists inappropriately, etc.).

Permission to Disclose Information Form

Those who receive assistance must complete a Permission to Disclose Information form. This signed form provides Thrivent Financial permission to use, disclose and publish the recipient's name and other related information about the recipient for the purpose of planning, conducting, advertising and communicating the results of an activity for the recipient's benefit.

If the recipient is a qualified not-for-profit organization, the form needs to be signed only if a person's name or likeness is published. This includes an employee or volunteer of the not-for-profit organization.

Permission to Disclose Information - Regular

Recipients (those who receive the help) must complete a "Permission to Disclose Information" form. This completed form provides Thrivent Financial for Lutherans with permission to use, disclose and publish the recipient's name and other pertinent information about the recipient for the purposes of planning, conducting, advertising and communicating the results of a fund-raising activity for the recipient's benefit (e.g., to publicize the fund-raising activity).

Note: If the recipient is a qualified nonprofit organization, the form does not need to be signed unless a person's name or likeness is published, which could be an employee or volunteer of the nonprofit organization.

A parent, guardian or other legal representative of a minor child or incompetent recipient (one who receives the help) must complete a "Permission to Disclose Information" form. This completed form provides Thrivent Financial for Lutherans with permission to use, disclose and publish the recipient's name and other pertinent information about the recipient for the purposes of planning, conducting, advertising and communicating the results of a fund-raising activity for the recipient's benefit (e.g., to publicize the fund-raising activity).

Note: If the recipient is a qualified nonprofit organization, the form does not need to be signed unless a person's name or likeness is published, which could be an employee or volunteer of the nonprofit organization.
